"All About Me" with IEP Summary: Template

Published: Oct. 31, 2024Updated: Nov. 1, 2024

To give teachers and service providers a head start in working with your child, we recommend creating an introductory "All About Me" document with a vision statement that outlines what your child needs for success. This will help your child’s care team get to know their strengths, interests, and goals as well as your (and their!) vision for their future.

Our "All About Me" template includes room for an IEP summary, which can include your child’s diagnosis, services, accommodations, supports, IEP goals, and contact information for the important people on your child’s care team; in other words, it breaks down the essential parts of your child’s IEP into an easy-to-read sheet.
