Printable Student Accommodations Sheet Template

Accommodations allow students to complete the same tasks as their peers but with some variation in time, format, setting, or presentation. Whether your child has an IEP or 504 Plan, it's important for teachers to know their accommodations so they can provide them. It's also important for your child to know their own accommodations so they can advocate for themselves in the classroom. This one-page sheet lets you share your child's accommodations with teachers or put in their binder as a reference.
Make a copy of this Google doc to create an editable version and add your child's list of accommodations.
