As Education Advocate Kelly Rain Collin tells us, “If it is not written, it doesn’t exist.” Here's a sample letter that shows how to document conversations with the staff, either in person or over the phone, which may be helpful to review during the next IEP meeting.

Dear Ms. Smith,
I am writing to follow up on our phone conversation today at 3:15 p.m. It is my understanding that I expressed concern that Mary has missed so many speech sessions since Mr. Alvarez has been on leave, and you made assurances that all the missed minutes would be made up by the end of the school year. I also expressed concern that the increased pull-out necessitated by the make-up minutes might result in Mary missing important instruction in the general education classroom. You then offered to meet with Mr. Alvarez and Mrs. Williams to draw up a schedule to ensure that the effect of the additional pull-out would be minimized. I look forward to hearing from you with the new schedule.
It is also my understanding that you will provide me with weekly service logs for Mary’s DIS services from now on.
If anything in this letter is missing or incorrect, please advise me as soon as possible.
