Sample Letter to Request an IEP Assessment

A parent can request an assessment of their child at any time. This sample letter can help you submit a written request for a special education assessment to be submitted to the school.

Dear Mrs. Martinez,

I am writing to request a comprehensive evaluation for Special Education services for my daughter, Mary Jones. Please provide an assessment plan for signature.

I am particularly concerned with Mary’s progress with reading and speech. A full assessment in all areas of suspected disability will be helpful.
