What documents do I need to sign for IHSS renewal?

The documents you will be required to fill out for your child's IHSS reassessment will depend on whether your child is receiving paramedical and/or protective supervision hours. It will also depend on your county and what your local IHSS office requires for recertification.
Remember that for any form you fill out, you should keep a copy or scan just in case IHSS loses it or doesn't record it.
