What does an IEP amendment look like?

If you request to amend your child's IEP, there is no standard format that the amendment has to follow. Some school districts will send a single page reflecting the change while others will send the full IEP document with the change included. An amendment should have the original IEP date and the amendment date.

Note that you do not always need to call an IEP meeting to amend an IEP. You could discuss a small change over email or the phone, for example, and request that your IEP team send you the updated IEP document.
