Create a one-page "IEP at a glance" to share with your child’s team, which can include your child’s diagnosis, services, accommodations, supports, IEP goals, and contact information for the people on your child’s care team.

An Individual Transition Plan (ITP) is a document created as part of the IEP to prepare a student for life after high school. The school should play the role of case manager in creating a collaborative team between the school and other government and public service organizations.

Once a student graduates with a high school diploma, they are no longer eligible for special education and related services through the school district. If a student has their certificate of completion, they can, by law, remain in the post-secondary program until their twenty-second birthday.
